A cross-sectional descriptive study in a sample constituted by 64 patients with diagnosis of syndrome of painful shoulder referred to the Service of Radiology and Diagnosis by Images "Dr. Theoscar Sanoja" was developed, to those who practiced radiological projections and ecografica exploration to them of the affected shoulder, by virtue of recognizing the information that offers each method for the definitive diagnosis of the cause of the syndrome, as well as characterizing the patients according to some intervening variables. Finding: predominance of the group of age between 47 and 55 years (43,7%), followed in frequency by those from 38 46 and 56 64 years with 15,7% each. The masculine with 54,6% predominated and the shoulder mainly affected was the right (71,9%). The common circumstance but of appearance of the pain was spontaneous (the 76,5%) and the chronic chronology prevailed with 85,9%. The radiological characteristics were normal in 90.6% of the patients, without demonstrating direct relation with the age, the sort, circumstance and the chronology of appearance of the pain. The ecográfica exploration reported pathologies in the 93,7% of the cases, predominating the acute tendinosis of the suprathorny (73,3%), the subacromiosubdeltoidea bursitis acute (45%), subacromiosubdeltoidea bursitis chronic (30%), chronic tendinosis of the suprathorny (16,7%) and the tenosynovitis of the long portion of the biceps brachial (10%). One concludes, with base in criteria of relevance and suitability, that ecography offers the determining data of the causes of the painful shoulder syndrome, not therefore conventional radiology, so that, in the protocol of the evaluation of patients with painful shoulder, it must settle down to the ultrasonografy analysis as the study of forward edge, independent of the age, the sort, the circumstance of appearance and chronology of the pain.
